2. What information do we process?
Depending on the care provided, we may process identification and contact details, appointment and correspondence data, billing and payment information, referral information, and information concerning your physical or mental health. For children and adolescents, we may also process relevant information about parents, guardians and parental authority.
3. Why do we process this information?
We process personal data to provide and document psychological care, communicate with you, manage appointments, comply with professional and legal record-keeping obligations, administer and invoice the services, handle complaints and safeguarding matters where applicable, and meet other legal obligations applicable to healthcare providers. 4. Your client record
A client record is maintained where required for the provision and continuity of care. It may include relevant intake information, treatment notes, professional assessments, correspondence and other information necessary for responsible care. Access is limited to persons who need access for the provision or administration of the care and who are subject to appropriate confidentiality obligations.
5. Confidentiality and sharing information
Information concerning your care is treated as confidential. We do not share information from your client record with third parties unless this is permitted or required by law, necessary for the provision or administration of care, or you have provided the required consent. 6. Security
Candoris Creative B.V. takes appropriate organisational and technical measures to protect personal and health information against unauthorised access, loss, alteration or disclosure. 7. Retention
Client records are generally retained for at least 20 years from the last amendment to the record, in accordance with the applicable rules for medical treatment records, unless a different retention period applies or longer retention is reasonably necessary. Other personal data is not retained longer than necessary for the purpose for which it was collected, subject to applicable legal retention requirements.
8. Your rights
Subject to applicable law, you may request access to your personal data and may ask for correction or supplementation of inaccurate information. In certain circumstances you may also request deletion, restriction of processing, data portability or object to processing. Rights relating specifically to the client record, including requests for destruction, are subject to the applicable healthcare legislation and professional rules.

De psychologische zorg wordt verleend door Tom Roz Eitan, Psycholoog NIP, in loondienst bij Candoris Creative B.V. Candoris Creative B.V. is de zorgaanbieder en is verantwoordelijk voor de verwerking van persoonsgegevens in verband met deze zorg.

2. Welke gegevens verwerken wij?
Afhankelijk van de verleende zorg kunnen wij identificatie- en contactgegevens, afspraak- en correspondentiegegevens, factuur- en betaalgegevens, verwijsinformatie en gegevens over uw lichamelijke of geestelijke gezondheid verwerken. Bij kinderen en jongeren kunnen wij ook relevante gegevens over ouders, wettelijke vertegenwoordigers en het ouderlijk gezag verwerken.
3. Waarom verwerken wij deze gegevens?
Wij verwerken persoonsgegevens om psychologische zorg te verlenen en vast te leggen, met u te communiceren, afspraken te beheren, te voldoen aan professionele en wettelijke dossierverplichtingen, de zorg administratief te verwerken en te factureren, klachten en eventuele veiligheidskwesties te behandelen en te voldoen aan overige wettelijke verplichtingen voor zorgaanbieders. Gezondheidsgegevens worden alleen verwerkt voor zover dit is toegestaan voor het verlenen en beheren van zorg en met inachtneming van het beroepsgeheim.
4. Uw cliëntdossier
Voor zover vereist voor de zorgverlening en de continuïteit daarvan wordt een cliëntdossier bijgehouden. Dit kan relevante intakegegevens, behandelnotities, professionele afwegingen, correspondentie en andere voor verantwoorde zorg noodzakelijke informatie bevatten. Toegang is beperkt tot personen die deze toegang nodig hebben voor de zorgverlening of de administratie daarvan en die aan passende geheimhoudingsverplichtingen zijn gebonden.
5. Geheimhouding en delen van gegevens
Gegevens over uw behandeling worden vertrouwelijk behandeld. Wij delen gegevens uit uw cliëntdossier niet met derden, tenzij dit wettelijk is toegestaan of verplicht, noodzakelijk is voor de zorgverlening of administratie daarvan, of u de vereiste toestemming heeft gegeven. Wanneer dienstverleners namens ons persoonsgegevens verwerken, worden waar nodig passende verwerkers- en geheimhoudingsafspraken gemaakt.
6. Beveiliging
Candoris Creative B.V. neemt passende organisatorische en technische maatregelen om persoonsgegevens en gezondheidsgegevens te beschermen tegen onbevoegde toegang, verlies, wijziging of openbaarmaking. De toegang tot cliëntgegevens is beperkt en op de systemen die voor de psychologische praktijk worden gebruikt, worden passende beveiligingsmaatregelen toegepast.
7. Bewaartermijnen
Cliëntdossiers worden in beginsel ten minste 20 jaar bewaard vanaf de laatste wijziging in het dossier, overeenkomstig de toepasselijke regels voor geneeskundige behandelgegevens, tenzij een andere bewaartermijn geldt of langere bewaring redelijkerwijs noodzakelijk is. Andere persoonsgegevens worden niet langer bewaard dan noodzakelijk voor het doel waarvoor zij zijn verzameld, met inachtneming van toepasselijke wettelijke bewaartermijnen.
8. Uw rechten
Voor zover de wet dit toestaat, kunt u verzoeken om inzage in uw persoonsgegevens en om correctie of aanvulling van onjuiste gegevens. In bepaalde omstandigheden kunt u ook verzoeken om verwijdering, beperking van de verwerking of gegevensoverdraagbaarheid, of bezwaar maken tegen een verwerking. Rechten die specifiek betrekking hebben op het cliëntdossier, waaronder een verzoek tot vernietiging, zijn onderworpen aan de toepasselijke zorgwetgeving en professionele regels.
